Community Support Team Nurses
Primary care medical nurses are part of a team of professionals dedicated to serving Providence Center clients experiencing severe and persistent mental illnesses. These nurses support their patients’ overall health and well-being by:
- Helping them to understand physical health issues like diabetes, obesity, heart disease and high blood pressure.
- Connecting them to primary care services
- Referring them to wellness programs like InSHAPE/InSHAPE Seniors, a Providence Center program designed to promote a healthy lifestyle.
